OSHA DOL Card Replacement

About This Course 

If your official OSHA DOL card has been lost, stolen, or damaged, you may be eligible for a replacement. To qualify, you must meet all of the following conditions: 

• You completed OSHA 10-Hour or OSHA 30-Hour Outreach training through 360training.com (OSHA.com’s provider) or one of its official partners. 
• You successfully completed the course within the past five (5) years. 
• You activated your original DOL card by calling the activation number provided with your card. 
• This is your first time requesting a replacement card. Per OSHA regulations, only one replacement card is allowed per student. If another card is needed, you must retake the course. 

IMPORTANT: Only the individual who completed the training can request a replacement card. Employers or third parties may not submit requests on behalf of students. 

Before submitting your replacement card request, log in to your student account and ensure your shipping address is correct. Your replacement card will be sent to the address listed on your profile at the time of order. 

How to Request a Replacement Card 

There are two ways to request your OSHA DOL card replacement:

2. Call Customer Support: You can also submit your request by phone with a support agent. 

If you call, be ready to verify: 

• Your full name 
• Training course completed (OSHA 10 or 30) 
• Completion date 
• Phone number and email 
• Shipping address on file 
• Details on how the card was lost, stolen, or damaged
