DOL Card Replacement
If your official DOL card has been lost, stolen, or damaged, you may qualify for a replacement card.
To qualify for a DOL Card replacement, you must meet the below criteria:
- Within the last five years, you must have completed your OSHA 10-hour or OSHA 30-hour Outreach training with 360training (OSHA.com's training provider).
- Your original DOL card must have been activated and verified by calling the activation number that was provided with the card.
- This is your first time requesting a replacement card.
OSHA regulations state that you are limited to just one replacement card due to loss or damage. If you need any additional cards beyond this limit you will be required to complete your OSHA Outreach Training again.
RECOMMENDATION Before placing your order we highly recommend that you log in to your student account and verify that the correct shipping address is associated with your student profile. This is the address where your replacement DOL card will be shipped.
